Health insurance members are required to take certain steps to have health insurance cards reissued or to receive benefits when a child is born. Described below are necessary procedures and ways to apply.
- * Fill out application forms using a black ball-point pen and submit the completed form (original).
To correct an entry, cross out the incorrect entry with two parallel lines and affix a correction seal.- Do not use a ball-point pen with erasable ink.
- Do not use correction fluid or correction tape.
