About the system for Voluntarily and Continuously Insured Persons
This system allows you to choose to remain an insured person of the Recruit Health Insurance Society after leaving employment.
If you want to remain a member of the Society after leaving employment
Procedures for the system for Voluntarily and Continuously Insured Persons
Fill out the necessary information on the Application Form for Certification as Voluntarily and Continuously Insured Person and submit to the Recruit Health Insurance Society by post within 20 days after leaving employment (must arrive during this period—postmarks are disregarded; applications by fax are not accepted).
- * Note that you cannot join this system if the application form is not submitted by the deadline.
If you want your family members to remain dependents after you leave your employer
If you want family members who were dependents while you were employed to remain dependents after you leave your employer, in addition to filling out items ⑪ and ⑫ on the Application Form for Certification as Voluntarily and Continuously Insured Person, fill out the necessary information on the Notification of Dependent (Change) of Voluntarily and Continuously Insured Person. Attach the various certifying documents and submit together with the Application Form for Certification as Voluntarily and Continuously Insured Person by the deadline.
- * If any documents are delayed, attach a note such as “Currently requesting ______; will send as soon as received.”
- Family members cannot be certified as dependents if they fail to meet the specified conditions.
- You cannot name family members as new dependents when applying to be a Voluntarily and Continuously Insured Person or during the period of coverage as a Voluntarily and Continuously Insured Person, except in the case of newborns in the family in which all other members are dependents of the Voluntarily and Continuously Insured Person.Check Family membership before applying.

If there has been a change in address, name, etc.
If there has been a change in address while being a Voluntarily and Continuously Insured Person, print out the Notification of Address Change of Voluntarily and Continuously Insured Person/Dependents, enter the necessary information, and submit to the Health Insurance Society. If there has been a change in name while being a Voluntarily and Continuously Insured Person, print out the Notification of Name Change of Voluntarily and Continuously Insured Person/Dependents, enter the necessary information, and submit to the Health Insurance Society.
- * Promptly complete change of address procedures if your address has changed. While you are a Voluntarily and Continuously Insured Person, the Recruit Health Insurance Society will send important information to your registered address, including notifications of changes in premium rates and information on family health examinations and complete checkups.
- * If your name has changed, be sure to submit the form with your health insurance card attached. A new health insurance card will be issued with your new name and sent by simple registered mail.

If you become an insured person under another health insurance plan upon re-employment
If you become an insured person under another health insurance plan upon re-employment, send the following three items to the Recruit Health Insurance Society.
Similar steps are also required if you become an insured person of the Recruit Health Insurance Society again.
- Notification Slip of Loss of Eligibility as Voluntarily and Continuously Insured Person (document sent with your Voluntarily and Continuously Insured Person’s health insurance card when you join the system)

- A copy of the health insurance card from your new insurer

<Return (refund) of insurance premiums after loss of eligibility>
- If you become an insured person under another health insurance plan upon re-employment, your insurance as a Voluntarily and Continuously Insured Person will be invalidated as of the date you acquire eligibility for your new health insurance.
- If you are due a refund of any insurance premiums already paid as a Voluntarily and Continuously Insured Person after completing the procedures for loss of eligibility, the Health Insurance Society will send a Request for Refund of Premiums of Voluntarily and Continuously Insured Person.
- Premiums already paid will be returned (refunded) based on your request. Fill out the necessary information and request a refund from the Recruit Health Insurance Society.
- * Premiums for the first month you became a Voluntarily and Continuously Insured Person will not be refunded.
- * If you joined another health insurance plan during the first month you became a Voluntarily and Continuously Insured Person, you must pay premiums for both insurance plans for that month.
- * The right to claim a refund expires after two years. Please request your refund promptly.
- If you use your health insurance card for examinations and treatment or complete checkups or other services after loss of eligibility, you will be billed for the entire amount of the relevant costs.
If you want to withdraw from the system for Voluntarily and Continuously Insured Persons
If you want to withdraw from the system for Voluntarily and Continuously Insured Persons for reasons other than expiration of the period as a Voluntarily and Continuously Insured Person, death, failure to pay insurance premiums, becoming an insured person under health insurance, Seaman’s Insurance, etc. upon re-employment, or becoming an insured person under the Medical Care System for the Advanced Elderly, submit the Application Form for Disqualification as Voluntarily and Continuously Insured Person to withdraw effective on the first date of the month after the month that includes the date on which this application form was received.
(Example: If your application form was received on January 15, 2022, you will lose your eligibility effective February 1, 2022.)
You cannot cancel this application once it has been submitted.
After the application form is received, you will be sent a Notice of Receipt of Application Form for Disqualification.
Return your health insurance card in the enclosed return envelope on or after the date of loss of eligibility.
- * If any insurance premiums are unpaid prior to the date of loss of eligibility due to the application, the date of loss of eligibility for failure to pay premiums will take precedence.
- * The unused portion of any insurance premiums prepaid in advance will be refunded.
