If you had medical eyewear prepared to treat juvenile amblyopia or other conditions

In some cases, you can receive health insurance benefits by applying to the Health Insurance Society for reimbursement after paying all costs to the medical care institution or other facility up front.

If you paid the entire medical care cost up front

Payment of benefits related to prescription eyewear prepared to treat juvenile amblyopia or other conditions

Required documents:

  • Copy of instructions from the insurance doctor in charge of treatment for preparation of prescription eyewear
    • * Usable for verification that preparation of the prescription eyewear was instructed to treat amblyopia or other condition
  • Copy of patient examination results (not required if test results appear on 1)
  • Receipt (original) for preparation or purchase of prescription eyewear
    • * Be sure to review the notes on the receipts below.
    • Must indicate the name of the child being treated.
    • The payment date must be on or after the date of the doctor’s instructions under 1.
    • The amount shown must be the actual purchase price including tax.
    • Must show a breakdown of prices for the frame, lenses, etc.

Amounts of benefits are based on the treatment costs stipulated under health insurance in Japan. See the “Description” sheet for more information on benefit amounts. If prescription eyewear was prepared again after payment of benefits of medical care expenses, you can apply for payment once again as long as certain conditions are met. See the “Description” sheet concerning the necessary conditions.
When submitting a renewal application, you will be asked to submit all of the above application documents just as for a new application.
Deadline: Within two years from the day after the date the costs required for treatment were paid to the medical care institution or similar facility (the day after the date treatment was received)

Notes:
  • The facility preparing the prescription eyewear must be certified by the Minister of Health, Labour and Welfare of Japan under the provisions of the Pharmaceutical Affairs Act.
  • Some municipalities provide subsidies for the copayment on preparation of eyeglasses as children’s medical care subsidies. Check with your municipality for more information. (The original receipt cannot be returned. Retain a copy if you need it for other applications.)
  • If you apply by notifying your Individual Number instead of your health insurance card code/number, you must submit separate documents to verify your Individual Number and your identification. See  for more information on the documents that need to be submitted for Individual Numbers.
